For New Patients

We want your first visit to be as comfortable as possible. We ask that you arrive to your first appointment 15 minutes early so that you have time to fill out some simple forms. This will allow us to spend as much time with you as possible. The forms include medical and dental history and insurance information. Please bring the names and dosages of all medications that you are currently taking or have taken in the last three weeks.

Your first visit will be a comprehensive exam which includes a head and neck exam, examination of the teeth and gums, and a full treatment plan. We may need to take some x-rays in order to complete the exam and make a proper diagnosis. If you have had dental x-rays recently, we recommend that you contact your previous dentist and request that they send copies to our office.
